<DIV><STRONG><FONT size=4>Thank you! I looked at this document and it is exactly
what I need! Unfortunately, from page 2 onward the text of the pdf file is not
centered properly in the vertical direction. The margin at the bottom appears to
be too large (about 2 inches) and there is no margin at the top --- there is
text right at the top edge of the page. In other words, the text begins at the
extreme top or the very top edge of the page. When I print the document the all
the text in the top one inch of the page is truncated and is not printed. So
there is missing text in the print out.</FONT></STRONG></DIV>
<DIV><STRONG><FONT size=4></FONT></STRONG> </DIV>
<DIV><STRONG><FONT size=4>Could someone please regenerate the pdf file so that
there is a one inch margin at the top of the page and also a one inch
margin at the bottom of the page so that it will print out
correctly?</FONT></STRONG></DIV>
