Samuel,

Thank you! I looked at this document and it is exactly what I need! Unfortunately, from page 2 onward the text of the pdf file is not centered properly in the vertical direction. The margin at the bottom appears to be too large (about 2 inches) and there is no margin at the top --- there is text right at the top edge of the page. In other words, the text begins at the extreme top or the very top edge of the page. When I print the document the all the text in the top one inch of the page is truncated and is not printed. So there is missing text in the print out.

Could someone please regenerate the pdf file so that there is a one inch margin at the top of the page and also a one inch margin at the bottom of the page so that it will print out correctly?
